The form is deliberately not designed to collect medical or health information. To prevent repeated spam submissions, the form temporarily stores a one-way identifier derived from the visitor’s IP address; it cannot be used by the clinic to recover the original address, records a 15-minute expiry and is removed by automated cleanup. Standard technical information may also be processed by WordPress, the hosting provider and essential security tools when you visit the website.
How we use it
